Welcome to Health Services for COCISD

We believe a healthy child is a better learner. With this in mind, our staff provides a variety of services for students, including first aid, health screenings per state regulations, development of individualized healthcare plans for school, or administration of prescription and over-the-counter medications with the proper documentation with proper consent. If you would like for your student to be provided health care service please complete the following form:

SB 12 Parental Consent Form

Without parent or guardian written consent the health service team will not be able to provide care per Senate Bill 12.
